Thank you for trusting us with your health care. The 436th Medical Group is a certified Primary Care Medical Home. Your PCMH team consists of doctors, providers, nurses, technicians, and support staff from one of the following assigned primary care clinics:
Pediatrics Clinic,
Family Health Clinic, or the
Warfighter/Flight Medicine Clinic.

PCMH certification is earned through continuous surveys conducted by The Joint Commission, the nation’s largest accrediting body for over 22,000 medical facilities across the country. What is a Primary Care Medical Home?
Five PCMH core attributes directly impact the care you receive at the 436th MDG:
Patient-Centered Care: Your medical home is a place of care where patients and family members are treated with respect, dignity, and compassion. This allows for a strong and trusting relationship between our patients and their care team.
Comprehensive Care: You’re assigned to a PCMH team consisting of your primary care manager (physician, nurse practitioner, or physician assistant), a nurse, medical technicians, and an administrative technician who partners with you to help provide all your health care needs.
Our clinic cares for Department of Defense beneficiaries enrolled in TRICARE Prime that are active duty service members, retirees, and their family members. Our PCMH teams serve in three co-located primary care clinics consisting of the
Family Health Clinic, the
Pediatric Clinic, and the Active Duty Clinics (
Warfighter and Flight Medicine). Each of these PCMH clinics offer comprehensive, outpatient primary care, to include acute care, preventive care, routine physicals, disease management, women’s health exams, and minor procedures.
In addition, the 436th MDG offers more specialized services to our active duty patients to keep the Warfighters entrusted to us ready to answer our nation’s call
The Physical Therapy Clinic is outfitted with state-of-the-art equipment and performs musculoskeletal evaluations and rehabilitative services to restore optimal physical function to our warfighters. The Optometry Clinic offers routine eye exams, contact lens fitting, and treatment for eye disease and eye injury. The Dental Clinic provides general dentistry, oral surgeries that require conscious sedation, and preventive dentistry. The Mental Health Clinic offers care to our active duty service members and limited services to family members with a focus on providing individual psychotherapy, psychiatry, support for new parents, and substance use disorder treatment services. The Operational Support Team (located at the base gym) offers education on many topics, including injury prevention, healthy lifestyle habits, improving sleep, stress management, and much more.
Superb Access to Care: Our clinic is open Monday-Friday, 7:30 a.m. to 4:30 p.m., including AMC Family Days; we’re closed on federal holidays. You can conveniently schedule an appointment 24/7 using the
MHS GENESIS Patient Portal. You may also call the 436th MDG Appointment Line at 302-730-4633 during normal clinic hours to speak to our friendly staff.
You can readily access care for non-emergent healthcare needs that arise after-hours or while traveling out of the local area by contacting the Military Health System Nurse Advice Line by dialing 800-TRICARE (874-2273) or by visiting the
MHS Nurse Advice Line website. The MHS Nurse Advice Line is available 24/7 by phone, web chat, and video chat.
Finally, the 436th MDG provides access to several services that add value to every visit you make to the 436th MDG, such as pharmacy, immunizations, laboratory, and radiology. The pharmacy offers the most affordable option for beneficiaries to get their medications. The Immunizations Clinic provides a wide range of routine and special vaccines (e.g., for deployers) to help prevent infectious diseases. Radiology provides common radiographic exams (X-rays). This saves you time by not having to travel to different facilities. It also gets the information back to your PCMH team quickly so they can guide your treatment.
What to do in an emergency: Call 911. The 436th MDG does not have an Emergency Department. Call us the next business day at 302-730-4633 to leave a message with your PCMH team, or send us a message on the
MHS GENESIS Patient Portal.
Systems-Based Approach to Quality and Safety: Our electronic health record, MHS GENESIS, is constantly undergoing modernization to empower our staff’s ability to leverage advanced health informatics. These updates implement clinical decision support tools that guide recommendations tailored to individual patients’ specific needs.
Coordinated Care: As a patient, you’re an active participant in managing your health. Our shared goal is to keep you as healthy as possible. Your PCMH team helps you coordinate all aspects of your care, to include wellness and acute visits, referrals for specialty care, obtaining results, and consultations with other providers for second opinions. Our Medical Management Team provides compassionate case management and disease management and has specialized knowledge of preventive health measures, outreach programs, and the Exceptional Family Member Program.
Choosing Your PCMH Team
With the exception of active duty service members, 436th MDG patients may change their PCM by calling Humana Military at 800-444-5445 or by visiting the
milConnect website. Active duty patients are assigned according to their unit for operational reasons, and are only switched in rare, extenuating circumstances using requests submitted through the 436th MDG’s Patient Advocate office.
Communication is Key
To receive the most benefit from time spent at the 436th MDG, we suggest the following important tips to ensure you can communicate with your health care team:
Sign up for the
MHS GENESIS Patient Portal. The portal enables you to view your health information, send messages to your PCMH team, request prescription renewals, view clinical notes, look up results, and make appointments. You can find a wealth of information on our
website about how to contact specific offices and obtain services throughout the 436th MDG. Finally, you can visit our
Facebook page to get updates about what’s going on at the 436th MDG, such as unexpected closures and special events.
Bring all your medications to your appointment (preferably in the original containers) and a list of any over-the-counter medications and supplements you’re taking and describe when, how, and how often you take each of your medications and supplements.
Write down any questions and notes you may have in advance. Feel free to bring a pen and paper to your appointments. If necessary, bring a friend or family member to your visit to help you remember answers to your questions or other details of the visit. Here are some questions to ask every time you talk with a doctor/provider, nurse, or pharmacist: What’s my main problem? What do I need to do? And why is it important for me to do this?
Provide a complete and honest description of your situation and concerns, even if it is difficult or potentially embarrassing to talk about. Let the team know if you have a current care plan and if you feel that you’re making progress towards your goals, or if you need further help or information.
Speak Up
Our clinic staff respects and encourages patients and family members to speak up! Our job is to make sure you’re comfortable expressing your preferences, needs, and values. Your responsibility is to convey them, as well as your concerns and questions. If you don’t fully understand, ask for further clarification. Your team should always use simple terms that are easy to understand and in a language you prefer.
